Claim Under  Motor Insurance

• Claim form duly completed and signed.
• Estimates of repairs and replacement of the affected vehicle/property from dealer/workshop.
• Photographs of the damaged vehicle/property showing number plate and damages distinctly.
• Statement of driver on the wheel at the time of accident and copy of his valid driving license book.
• Notification of the loss to police & their report.
• Copy of whole set of valid registration book of the damaged vehicle.
• Copy of Route permit & Janch Pass of the vehicle valid at the time of accident.
• Load chalan /Passenger chalan of the vehicle related to the time of accident.
• Bill & Cash Memo related to the repair and replacement of the damaged vehicle/property.
• Other claims related documents required under merit basis of loss.

In case of Third Party /Passenger/Staff‘s Bodily Injury

• Complete sets of medical papers including doctor’s prescriptions, Medical cash memos, Pathological reports, Admission & Discharge Ticket & other treatment related medical documents.
• Aattested copy of Milapatra and all other claims related documents as needed.

In case of Third Party /Passenger/Staff‘s  Death

• Copy of Abhiyog Patra.
• Letter issued from CDO office to release compensation.
• Attested copy of Court Decision, if any.
• Postmortem Report and Death Certificate, Ghatanasthal/Lash Janch Prakriti Muchulka.
• Copy of Legal Heir Certificate and Citizenship of deceased & legal heir.
• Claim letter served upon you by third party.
• Copy of Payment Receipt/Milapatra and all other claims related documents as needed.

 
Claim Under  Personal Accident/Health Insurance

• Claim form duly completed and signed.
• Complete sets of medical papers including doctor’s prescriptions, Medical cash memos, Pathological reports, Admission & Discharge Ticket & other treatment related medical documents.
• Medical certificate duly filled and signed by attending doctor.
• Date of duty resumed by the employee after the accident including copy of approved leave form.
• Copy of Monthly Salary/Payroll of the injured person.
• Doctor’s prescriptions & cash memos for the medicine purchased.
• Hospitalization Reports including Discharge Certificate with Pathological Reports including X-ray Plates.
• Copy of Autopsy (Postmortem) Report.
• Copy of Death Certificate.
• Copy of Legal Heir Certificate and Citizenship of deceased & legal heir.
• Copy of notification of loss to police and their investigation report.
• Copy of Court Decision, Ghatanasthal Muchulka..

• Stock book of the affected property prepared prior to loss.
